We at Hy-Tech Weight Loss want to give you every opportunity to succeed, and that includes extra incentives. We are offering you an opportunity to receive one-half (50%), of your program cost (program cost is the cost listed on your Enrollment Agreement under "Program Cost") back as a part of our performance-based rebate incentive program. All we are asking you to do is reach your goals!

Rebate Eligibility Requirements

Receive, read, complete, and sign an Enrollment Agreement
Receive, read, complete, and sign a Performance Based Rebate Form
Attain your weight loss goal:
1. On your final weigh-in at the end of your program you can weigh no more than five (5) pounds OVER your goal weight in order to receive this rebate.
2. You may weigh less than your goal weight at the end of the program and still receive this rebate.
Complete the Full Program
1. Make visits to Hy-Tech Weigh Loss Centers in strict accordance with your program and contract.
2. Keep diet and exercise records in strict accordance with your diet program plan provided to you by Hy-Tech Weight Loss.
3. You will not be eligible for this rebate if you reach your goal weight prior to the end of your program if at the time your program ends you have not maintained your weight loss or are not in compliance with number 3 above.
At the end of your program, your balance with Hy-Tech Weight Loss must be paid in full. Your balance due to Hy-Tech Weight Loss must be zero ($0) and have no delinquencies on your payment records with Hy-Tech Weight Loss.
You must adhere fully and completely to the Enrollment Agreement including all promises, agreements, and warranties made therein.
Upon completion of your weight loss program with Hy-Tech Weight Loss, return your signed copy of this Performance Based Rebate form along with any additional records, documents, and other forms to the center where you first signed up with Hy-Tech Weight Loss. At that point you and a counselor will review your records, diaries, payment history, and other applicable documents in order to determine if you qualify for this rebate.

REMEMBER! It is your responsibility to keep accurate records of your payments and all other necessary records and documents that illustrate your compliance with Numbers 1-6 above. Please keep your copy of this document and all other records and documents pertaining to your program with Hy-Tech Weight Loss in a safe place where you readily know where they are. If your documents and records are lost and/or destroyed we may have no way of verifying your compliance with the requirements of this rebate.

Only one (1) performance rebate is available per participant per program period. Only the program participant named and signed below may be eligible for and receive any rebate. The rebate amount is only 50% of your program cost as listed on your Enrollment Agreement ONLY; your rebate will not include taxes or other fees. Hy-Tech Weight Loss reserves the right to require any and all applicable paperwork from the applicant to confirm compliance with the rebate eligibility requirements. Hy-Tech Weight Loss is not responsible for any participant supplied documents, records, and/or contracts that are mutilated, late, lost, or illegible. Hy-Tech Weight Loss reserves the right to change, modify, or cancel this promotion at anytime without notice. However, any participant who has signed a Performance Based Rebate Form prior to any time that Hy-Tech Weight Loss cancels or suspends this program will still be eligible for any rebate they may qualify for at the end of their weight loss program. Please allow 8-10 weeks to receive your rebate check.
